<p>Immunodeficiency Syndrome or AIDS is a collection of symptoms and infections resulting from immune system-specific damage caused by the human immunodeficiency virus. The late stage of the disease leaves individuals prone to opportunistic infections and tumors. Although treatments for AIDS and HIV exist to slow the progression of the virus, but there is no known cure.</p>
<p>There is currently no vaccine or cure for HIV or AIDS. The only known method of prevention is based on avoiding exposure to the virus or, failing an antiretroviral treatment immediately after a major exhibition, is known as post-exposure prophylaxis.</p>
<p>Well current treatment for HIV infection on antiretroviral therapy is highly active, also known as HAART. This has been very beneficial to many HIV-infected individuals since its introduction in 1996. Current optimal HAART options consist of combinations consisting of at least three drugs from at least two types or &#8220;classes&#8221; of anti-retroviral agents. Typical systems consist of two nucleoside reverse transcriptase inhibitors plus a protease inhibitor or reverse transcriptase inhibitor non-nucleoside.</p>
<p>HAART allows the stabilization of the patient&#8217;s symptoms and viremia, but not cure the patient HIV, nor alleviates the symptoms, and high levels of HIV-1, often resistant to HAART, return once treatment is stopped. Moreover, it would take more than the life of a person to be free of HIV infection and antiretroviral treatment. Despite this, many people infected with HIV have experienced remarkable improvements in their overall health and quality of life, which has led to the collapse of HIV-associated morbidity and mortality.</p>
<p>In the absence of HAART, the progression of HIV infection to AIDS occurs at a median of between nine to ten years and the median survival after developing AIDS is only 9.2 months. HAART is thought to increase the survival time of between 4 and 12 years. This average reflects the fact that for some patients and many clinical cohorts this may be more than fifty percent of HAART patients get much less than optimal results. This is due to a variety of reasons such as intolerance to medication or side effects, prior ineffective antiretroviral therapy and infection with a drug resistant strain of HIV.</p>
<p>However, the lack of adherence and non-persistence with antiretroviral therapy is the main reason most people do not get any benefit and develop resistance to HAART. The reasons for non-compliance and non-persistence with HAART are varied and overlap. Major psychosocial problems such as poor access to health care, inadequate social support, psychiatric illness and substance abuse contribute to non-adherence. The complexity of these HAART regimens, either by the number of pills, dosing frequency, food restrictions or other issues along with side effects that create intentional non-adherence also has a major impact. Side effects include lipodystrophy, dyslipidaemia, insulin resistance, increased cardiovascular risks and birth defects.</p>
